NEW prescription pads required – Effective January 1, 2019
 

Texas physicians will be required to use new schedule II prescription forms beginning January 1, 2019. The new forms are now available from the Texas State Board of Pharmacy (TSBP). After January 1, official schedule II prescription forms that were issued before September 1, 2018 will no longer be valid.

To order new prescription pads, please visit the TSBP website.

It may take up to 30 days to ship prescription forms once they’re ordered, so please plan accordingly
